Living Well with Type 2 Diabetes’ Group Education program

Barwon Health's Newcomb Community Health Centre is running the ‘Living Well with Type 2 Diabetes’ Group Education Program in November 2017.

The program is targeted at adults with Type 2 diabetes and their family/carer/s. The program will run for the five consecutive Wednesday’s in November: 1st, 8th, 15th, 22nd and 29th from 1pm to 3.30pm.

The aim of the group is to help consumers self-manage their diabetes.

Various health professionals including a diabetes educator, dietitian, podiatrist, social worker, physiotherapist and dental assistant will present information about:

  • eating well
  • being active
  • diabetes medicines
  • checking blood glucose levels
  • the Diabetes Annual Cycle of Care (checklist).

In addition, discussion can center on:

  • what the experts recommend
  • making choices that help manage diabetes 
  • how to keep on top of your diabetes
  • how to prevent diabetes complications
  • anything that might be causing worrying the person with diabetes about his/her diabetes.
